Definitely a few times. I play single player most of the time though so at most the game goes to pause and either I fail my mission or lose a life. I have a USB cable that fits into my One's controller though so I can fix it pretty fast without having to run to grab batteries.

Just kidding tho.
This happens to me a bunch. PS4 will sometimes display two bars of battery, them controller goes dead just ten min later. Always keep a USB cable close by.

With the WiiU, all the time. Shitty ass battery.
Then a handful of times on the 360 during marathon Halo sessions. Just kept spares ready. No problems on PS4 or X1. They both have longer battery life though and I toss 'em on the charger after each session.

All the time. Game pauses, I get a different controller or hook up the cable, start playing the game again.

Just last night while playing CoD multiplayer actually. Miraculously I didn't even get killed while fumbling in the dark for my USB cable as my online avatar stood motionless like a potato.
