Spike and Anya were huge additions to Buffy, as were Wesley and Fred to Angel. Frank Vincent has meshed well on the Sopranos, as has the guy who plays Johnny Sacrimoni. Tina Majorino (Mac) on Veronica Mars next year (if she becomes a regular) will be great.
I'm guessing Lloyd on Entourage doesn't count, but he was one of the best parts of the second season.

How about Heather Locklear's addition to Melrose Place? That was a MAJOR improvement!

The Best:
Fred and Lorne on 'Angel.' They both came into the show after it was pretty well established (I consider Wesley and Gunn original cast members because they came in so early on). If it wasn't for Fred and Lorne, 'Angel' just wouldn't had the heart that it was able to develop over seasons 3-5.
Jack on 'Dawson's Creek,' he was introduced around episode 3 or 4 in the second season, and didn't become a major player until well over halfway through the second season, but in season 3 the Jen (michelle Williams)/Jack dynamic is what kept me coming back episode after episode for the remainder of the series, and honestly I think he is the best example of a last coming character meshing so well into the original cast group. Its kinda funny because he was always suppose to be a minor character, and his sister was suppose to join the core group of friends, but in reality she always stayed the minor character.
Lois Lane on 'Smallville,' she was main reason I started watching teh show again in season 4, after giving up on it in season 3, and I believe that bringing her into the show was one of the smartest things the producers ever did.
The Worst:
ER - I'm sorry but there are only two characters on the show presently that I actually like (not including Weaver), and I am probably in the minority but i have NEVER LIKED LUCA!
OC - The 'new kids' from season 2, all of them sans Summer's boyfriend bugged the living hell out of me (I could stand Alex, but not that much), and it was no suprise that they where all gone by the end of the season. Next year after already losing Luke and Anna early on with Marissa gone (and her sister becoming a major player with Taylor) I think the OC has finally hit rock bottom.
Growing on me:
Rachel on 'One Tree Hill,' call me crazy but this last season with her has been really interesting and entertaining. Yes she can give Dan a very earned run for his money in the "OMG they need to be slapping severl hundred times over' category, but I still really like where the writers are going on her
Hannah on 'everwood,' it took me awhile, but I really started to like Hannah and everytime the opening credit came on I wished that she would finally appear in them (but never did, they should of made special credits for the finale, a la Tara on BUFFY), but as much as I really liked Hannah, I HATED Jake the same. Not because he was Andy's rival, but because he was bland and boring.

Joshua Malina to the West Wing. He's no Sam Seaborn but I love Will Bailey. One of my favorite actors since Sports Night.

Cheers probably did the best job of adding new/replacement characters in btwn Woody, Rebecca, Frasier, and Lilith.
For dramas, Dick Wolf did a very good job of of replacing characters in Law & Order up until the attrocious Rohmbot debacle.

Oops! That's a big one. Yeah, Oz is definitely apart of that list. Oz rules. Wesley definitely became one of the best characters ever.
I didn't mind Wesley on Buffy though. He was a bit more uptight at first, but at least he didn't kick Giles out and completely take over when he could have. He eventually kind of became a part of the team and by the end just wanted to help out.
So, he was better on Angel, but he wasn't too bad on Buffy, just a bit more uptight. His kissing scene with Cordelia on Buffy was an instant classic funny moment due to the awkwardness of it. When he joined Angel it was really nice just to see a familiar face and he brought the funny before he became hardcore.

Angel did indeed appear in the Buffy pilot, but during the first season he was a recurring guest star.
David Boreanaz wasn't added to the cast until Season Two.

I didn't notice anybody mentioning Christopher Meloni as Chris Keller on Oz. I think he was a very important addition to that show.
Forest Whitaker as Kavanaugh on The Shield has really turned things up a notch.

Ever wonder how DS9 would have turned out if Forbes agreed to continue playing Ro, and therefore having Kira simply not exist?
It's the same exact situation with Max Perlich, recently seen in the season finale of "My Name Is Earl"...his Whistler character from a Buffy episode was to be alongside Angel and Cordelia for ANGEL's first season, but, like Michelle Forbes - didn't want to commit to a series and therefore Doyle was created. In both cases, the new characters have the same responsibilities as the original characters, but without the established past history.
You just have to wonder how things on both shows would have played out.

I hate it when I run across a thread like this and all the good answers are taken . So Ill just add another vote for Kelsey Grammer as Frasier Crane and a nod to Woody Harrelson as Woody Boyd (even though in a way he just replaced the Coach character). Also for Buffy (which Im only 2/3rds through season four currently), David Boreanaz, Seth Green, and James Marsters.

Although I think it needs a little explanation. Jefferson was a great addition, but he wasn't quite as good as the original Steve. However, he was better than the season 4 Steve that lost his job, and of course left Marcy. And Jefferson certainly improved the dynamic of the show from where it was with Marcy as a single woman, which just wasn't working. Jefferson improved her character along with being a great partner for Al.
Other good ones:
Butters on South Park - I don't know how animated shows fall on here, but Butters is just a great character and really adds a lot to whatever episodes he's in.
Alan Shore on The Practice - A character so good, he got a spinoff, and it only took one season
