how much does it usually cost to send one (musical) CD, including postage and packaging, to somewhere in the US?

do you ship it in an envelope or some kind of box?

One CD would be faster and cheaper to send 1st class (probably 1.06).

First class shipping ranges from $1.06 to $1.29 in a padded mailer. Cardboard mailer might be around $1.48. You can buy both the padded, sealable mailers or the carboard "box" mailers for around .25 each if you buy in bulk. If you only need to mail one or two CDs, Walgreens usually runs occasional sales on padded envelopes...3 for $1.

If you're mailing from home, two 37 cent stamps will be enough, assuming it's just the CD in a paper sleeve. If you include a jewel case then it'll be 3 stamps.
