I read that Brian De Palma wanted Robert De Niro to play Al Capone in The Untouchables. Since he wasn't available at that time, Bob Hoskins was hired. When De Niro's schedule opened unexpectedly, De Palma offered the role to De Niro and paid Hoskins full salary for his inconvenience.

Don't feel too bad for him. He got to bang Charlize Theron for years!

I don't remember who the actress was, but in the Almost Famous commentary, Cameron Crowe talks about how one of the "band aids" was supposed to have a bigger role but due to problems with her (not even showing up, I think) her scenes were given to one of the other "band aid" actresses.

Wizard of Oz stuff:
-Buddy Ebsen was the Tin Man but got hospitalized from the makeup.
-Forgot her name, but a leading candidate for the Wicked Witch remained too pretty for the role (The goth makeup seemed to increase her beauty!)

Eli Sunday was supposed to be played by a separate actor (Kel O'Neill) in There Will Be Blood. For whatever reason (and the reason supposedly is that he got freaked out by Daniel Day-Lewis) Paul Dano took over the roles of both Sunday brothers apparently just to confuse the hell out of people seeing it for the first time.

Mel Gibson was slated to have a cameo in The Hangover 2 (as the tattoo artist) but was supposedly fired since certain talent didn't want to work with him.

And maybe not "fired" per se, but there's a pretty infamous story of Adrien Brody being cast as the star of Terence Malick's The Thin Red Line only to discover at the premiere that he was almost completely cut out of the movie.

The lead terrorist in True Lies was 'fired'.
