Film fatalities- Dying to be in the Movies
I'm not sure if this has been posted before, but it's an interesting piece.
A list of movie set casualties...some of these are pretty obscure, some of course are well known (Vic Morrow, Brandon Lee etc). Oh and lots of stuntmen..

Totally worries me now about what I may face in my career in the film industry. WTF about the Outer Limits..it's literally changed my view of Spielberg now...

It wasn't his segment and it wouldn't surprise me if he had no knowledge of the illegal child labor.
John Landis on the other hand....
Not the same thing but didn't some child actor kill himself because his scenes were cut in Harts War or something.

Good lord, no shit. I never saw that and don't even know what it's about. He was 27 and thought his life was over acting-wise? What a tool. I know people 10 years older than that who didn't have half the opportunities he did and they're plugging away like champions and at least earning a living as actors.

You may want to seek out a book called "Outrageous Conduct". It pieces together how the accident occurred, the parties involved, and chronicles the trial afterward. Spielberg was the Executive Producer and wasn't really involved in the Landis segment as he was directing his own chapter of the film at the time. However, his team of lawyers went to great lengths to distance him from the trial as well as downplay his producing role in the film etself.

I wouldn't worry. Seriously, unless you're going to become a stunt man. Even then, they take safety very very seriously. There are more rules and safety precautions than you could read in a year on film/tv sets. You hear about set accidents for the same reason you hear about plane crashes: they are very very rare.

Actor Eric Fleming drowned in Peru in 1966 while shooting a scene for something for TV called "High Jungle."
Here's what it says in his IMDB bio:
"During the shooting of location shots on the Huallaga River on September 28, 1966, Fleming dove from a dug-out canoe after paddling it beyond the rapids. His body was lost in the turbulent water and was not recovered until three days later."
Fleming was the star of the TV western series, "Rawhide," in which his partner on a cattle drive was played by Clint Eastwood. Fleming starred in such '50s sci-fi as CONQUEST OF SPACE (1955) and QUEEN OF OUTER SPACE (1958) and the vampire western, CURSE OF THE UNDEAD (1958).

They Died With Their Boots On (1941)
Three horseman perished during the cavalry charge, one of whom was extra Jack Budlong, whose horse tripped as he rode alongside Errol Flynn. As he fell forward, he had the foresight to toss his sword ahead of him. Unfortunately, it landed handle down and stuck in place. Sound familiar, Charge Of the Light Brigade Fans? Jack was impaled on his own sword, and died in a L.A. hospital a few hours later.
