Requiem for a Dream is a great movie, but I wouldn't really classify it in the same category as Fight Club or Donnie Darko, as its not a surprise ending or mindfuck movie. However, I was thinking Pi might be more suited to what you are looking for (its by the same guy that did Requiem) and definitely on the bizarre side of the line.

I'm all about the same kind of movies that you're looking for. Some good suggestions so far. Definately the ones Jay G mentioned. Also, I'll include:
The Last Temptation of Christ
Twin Peaks
Psycho --just about any Hitchcock (although not as hip with the mindfuck)
In the Company of Men

Most of these are second/third/fourth votes, not all are "mind fucks" but have some general themes and moods in common:
American Psycho
Requiem for a Dream
Dark City
Mulholland Drive
The Wicker Man
The Butterfly Effect (terrible rip off of DD, but a lot of people seem to like it)
Serial Experiments Lain (anime,TV series, 13 eps - check it out if you like Lynch or Cronenberg)
eXistenZ
Videodrome
Lost Highway
A Tale of Two Sisters (Korean)
Tokyo Fist (Japanese)
... numerous others I can't quite think of right now.
And while Eternal Sunshine of the Spotless Mind is a terrific lil flick, I don't really fit it in the same catagory, nor do I any Kaufman's work. They're all really good (yes I even liked Human Nature) and all, but they actually seem a bit more grounded than the aforementioned. Of course, as per usual, I'm probably missing the boat on what this guy is looking for
