It usually means it was mixed live, and that each song has contiguous lead-ins and lead-outs which have been beatmatched.

I'd add that it's not necessarily mixed "live" - it could be done in a studio, on computers, etc. I have a whole bunch that aren't mixed live and you can tell - they are just too perfect and would require multiple turntables, samplers and such.

Check out 'Journeys By DJ' series the COLDCUT mix is one of the best.
