As everyone seems to agree, and having suffered through this myself, the nail should be drilled so the blood can be drained. Then eventually the damaged nail will be pushed out as the new nail grows, assuming that the root or matrix isn't seriously damaged.
Go see a doctor or use a non-sterile implement and risk something more severe.
Remember that something as simple as an untreated hangnail can lead to death. See blood poisoning/sepsis.

Only drill through your toenail if it's throbbing with pain, otherwise just let it be and it'll heal itself. If you do decide to drill I'd recommend a straightened out paper clip. Heat that bastard red hot and then slowly push it into the nail.

Dropped a 75 lb dumbbell on my big toe once... blood pooled and hurt like a sonovabitch. Until I spent a $35 copay to visit the doc in the box, who used a small laser to burn a tiny hole in my toenail. The pain stopped instantly once the blood could get out. I did end up losing the toenail, and it still has some issues a few years later.
