What are the grammar mistakes that bug you the most?

The two that bug me quite a bit are the use of the word loose when they mean "lose" and people not properly using than and then.

Joking aside (), the misuse of there, their and they're probably bother me the most because they're the most common ones I see.

Here are a few:
they're, there, their
you're, your
loose, lose
passed, past
waived, waved
Incorrectly spelled words at all times!!!
Very poor grammar, for example from people who are using English as their second language and do not always remember that English is an S-V-O language, so they write some sentences using S-O-V word order. Funny, I can listen to this error without problems, but reading this type of error really bugs me, possibly because I am feeling that if you're writing then you should take special care.

I can't get mad at incorrect usage of "its" versus "it's" ... since I make that mistake all the time (even knowing the rule)

brainee - I love the videos you posted.

I also mess up with it's and its and oftentimes do not even notice when there is a mistake, but if somebody is writing a book then it's irritating to read/see it. When I post my webpages I always try and remember to double check all of my it's and its for correctness - I don't want people to see that type of mistake by me in a professional or even semi-professional type of setting. It's uncalled for!

What's maddening is how you can never find every one.

I got a report today with perhaps the best typo I've yet seen:

Proofread carefully to make sure that all spelling and punctuation
is not up to professional standards.

I think I am guilty of doing that from time to time.

My irritation is double superlatives.

"most strongest"
"most tallest"
"more harder"
"more uglier"

I tend to give the benefit of the doubt to people on message boards because I know many of us post from our iPhones or other portable devices that either have tiny buttons making it hard to type or autocorrect that changes something you originally typed correctly. What I hate is people who don't use punctuation at the end of their sentences. It makes their whole post incredibly difficult to read and comes off sounding like some weird stream of consciousness type post.

I've noticed in recent online discussions that many people get the words "wary" and "leery" mixed up (they mean practically the same thing) and incorrectly combine them as "weary" (which doesn't have the meaning they're going for).

For example: "I'm very weary of strangers who offer me candy from vans."

All of the above, but one that bugs me that is so far unmentioned is:
Peak/peek/pique.
"Sneak peak of the sequel!"

My phone autocorrects your to you're half the time, and it or its to it's. Arg.
