I hate people who say "what?" when you're never even half way done with the question yet.

Stop asking questions. It's an indication that you aren't smart enough to figure things out without being told by someone else.

If a tree falls in the middle of a hearing impaired person and nobody is around to hear it... no, wait.

I'm a horrible listener, and on top of that, years of playing in loud rock bands have killed my hearing, so I almost never catch something the first time it is said to me.

I used to have a fried who specialized in dealing with this.

He would almost always make a side (usually rude/tacky) comment to people in the normal course of conversation that would go completely over their heads.

Depends on the situation - If you already have my attention, then only once. If I don't know you're talking to me yet, probably at least twice.

Only once.

Mrs Danger claims that it's many, many more, but I've never noticed anything like that.

Like girlnextdoor, it depends on the would-be listener. Most people hear me the first time, but I have a cow-orker who has to have everything repeated at least once.

The wife is usually twice when starting a conversation. If we're already talking she's usually OK.

But, I can even say "Hey" first, wait a second, and she'll still say "what?"
