Ben Folds Live has a DVD with 6 or 7 songs from a concert on it... Including at least three songs that are not on the CD (Eddie Walker, The Ascent Of Stan & Song for the Dumped).

I'm going with Primus "Animals Should Not Act Like People" as well. Maybe it shouldn't count, but it's the only "bonus" DVD I have from a CD, or is it a bonus CD from a DVD? Who knows, but that's my vote anyway.
