The Morning After with Dick Van Dyke from 1974.

I remember seeing this in '74 (aged 14) and it was incredibly powerful, especially the ending (which is sadly all I really remember in detail about it):

Spoiler:
Van Dyke, a successful businessman, has messed up his life with his drinking but has tried to stop, to no avail. At the end he accepts his true nature and sits down in an alley with a bottle, presumably to live his life as a homeless drunk.

Not on video or DVD, along with lots of other great TV movies, but this one has the additional hurdle of including Yesterday by The Beatles at the end so music rights are probably an issue. I personally think they could use another song (the final scene is without dialog) or another recording of the same song (still an issue but probably easier/cheaper to do than the original version) without damaging the scene.

The 2 Best Hollywood ever made where IMHO, Billy Wilder's The Lost Weekend, even though he was forced by the studio system to tack on a completely ridiculous ending, where our hero finally "gives up" due simply to the love of "a good woman". I'm sure anyone who is or has been a drunk (like myself) knows this would never happen in real life!

The other is Blake Edwards' Days of Wine and Roses, with its much more "realistic" ending. The acting by the "drunks" in both these movies is superb, with the first, Ray Milland actually winning an Oscar as Best Actor of the year.

IMO, both these movies have yet to be bettered in showing not only the effects of alcoholism on the "drunk" but on everyone else around them.

Wow, I can't believe no one has mentioned My Favorite Year yet. I think only Dudley Moore has been able to rival Peter O'Toole for the title of best on-screen drunk on a comedy in my lifetime.
