Here's an oldie but a goodie: Dick Cavett and James Murtaugh could easily be confused for identical twins... or at least they could back in the 70s and 80s (age has made them look quite different these days). Hell, they even sounded the same.
Eddie Murphy even commented on it on SNL or in one of his stand-up routines... something like "screw Dick Cavett... screw that guy that looks like Dick Cavett..."
Oh - and Ray Wise (Twin Peaks, Reaper) and DJ Paul Oakenfold were separated at birth.

If someone can say earlier one that everybody recognizes of great age difference, Helen Hunt and Lele Sobieski, then I can say one that's always been apparent to me...Josh Hartnett looks exactly like a young Tommy Lee Jones, especially when he parts his hair short. I've been looking for the day that they play father and son or even the same character at different stages in life.

A few I've always noticed have been mentioned already. I definitely used to think Skeet Ulrich was Johnny Depp in As Good As It Gets, before Depp got so famous. I thought that for a couple years.
I noticed Eli Roth and Sylar from Heroes a while back.
One I've always thought but haven't heard people say is Mickey Rourke in late 80's around Angel Heart Days and Bruce Willis around the time of the first Die Hard. They used to look just alike to me before Mickey fell apart and Bruce lost his hair.

Yes, I've always thought Natalie Portman and Keira Knightley looked similar. That's why in Star Wars Episode I Keira played Natalie's decoy in her princess make-up.
