Just watched this last night:

Director Agustín ("In A Glass Cage") Villaronga's:
"El Mar"

Spoiler:
The scene where the two young boys are fighting in the cave ultimately, one of the boys precedes to bash in the kid's head on a rock and to make sure he is definatley dead, brandishes a knife and stabs him in the throat four times

I had tuned that out of my memory, thanks for bringing it back.

I don't know if anyone has mentioned this scene yet, but in City of God the gangster known as Lil Z corners two kids (children, mind you) who have been stealing from his territroy. While they are crying, he makes them choose whether or not they want to be shot in their hands or their feet. They tearfully hold out their hands, and Lil Z proceeds to shoot them in their feet anyway. Then, he actually plugs the older kid and leaves the other one to bleed to and cry over his body.

Tears Of The Sun, everyone!

A mediocre "let's do a John Wayne war film set today" film, but in the "news footage" of the baddies rolling through town killing all who resist - there's some very, very grisly images, and the fact that they're shot realistically with no "filmish" atmosphere makes it very disturbing.

Then we're put back into the film, which wastes any punch the "news footage" had.

I remember seeing this as a kid and crying at that scene because I felt so bad for Goldblum's character.

The last ten minutes of Requiem for a Dream really messed me up.
Ed Norton's blood squirking bashing scene in Fight Club also got to me.

But the one scene that completely takes the cake is a deleted scene from Casino which often airs unedited on Encore:
Joe Pepsci wants the name a crime boss but the guy he caught refuses to talk, in order to force him he puts the guy's head in these huge vice grips (or some kind of tool) which squeezes the guy's head at his temple until his eyeballs pop out.

We the audience were spared the scene where Goldblum's character during his final transformation has a giant fly leg emerge from his body and in total desperation tries to bite it off, word was that the scene really was OTT and utimately fell to the cutting room floor. This movie really needs to be rereleased as a DVD Special Edition.

The final take over of Malkovich in Being John Malkovich .

But, for me, nothing beats the hobbling scene in Misery .

Does this really air uncut on Encore? I'll have to look out for it.

Absolutely. I got sick to my stomach watching this part. Great film. But very very disturbing scene.

I agree with everyone that mentioned "Hannibal". The end of course was the worst. But it was pretty gross throughout. Can see why Foster passed.

Also agree "Seven" and "American History X" (I can still hear the crunching in my head).

Really suprised no one has mentioned the crucifx scene in "The Exorcist". Sure the movie is dated. And way more shocking things have happened in the movies since. But it was a landmark shocking moment for me. Hell, the whole movie was (I was 13 when I first saw it).

Also, I thought "To Live and Die in L.A." had several disturbing scenes.

This was the first thing that popped in my brain...

Its not a gory or 'flashy' scene but its mentally disturbing to think about...

The pen going and twirling slowly in the ankle in "Evil Dead".

But the most disturbing scene for me has got to be the ending of "Invasion of the body snatchers" (1978). If you've seen it, you know what I mean

The footage of Columbine in Bowling for Columbine

Cannibal Holocaust - the large turtle being slaughtered

These two are the most disturbing for the same reason - it really happened. The second example I have absolutely no tolerance for because it was done for entertainment purposes.
