For me, I'd say...

Certain scenes from certain movies are disturbing (aforementioned scene in Blade or Se7en ending), but overall I'd say maybe The Crow or Amor Es Perros.
Notting Hill gets an honorable metion...

Good point, I dont' own it but the violence and menacing behavior throughout that film are pretty intense.

The funny thing is, I own the majority of the movies being mentioned here. I think I need a prescription for Prozac or Zoloft or whatever.

I totally agree with a previous poster about Dead Ringers. That film is just painful to watch. Actually, most of David Cronenberg's movies are disturbing, and I think a big part of it happens to be the music. Howard Shore's scores just have this abysmal tone to them... Next to Dead Ringers, Cronenberg's most troubling movie is The Brood, IMO, followed by Crash and Scanners.

I don't own it and haven't seen it in years, but Leaving Los Vegas really put me in a depressed mood. Interesting flick, but another big downer.

I am a fan of disturbing cinema, although for me there has to be an emotional impact besides just gore or pretend gross-out (for example, 8MM).

For me, however, films (or scenes from films) such as You Can Count on Me and Do The Right Thing are certainly upsetting although they are not graphic in their nature.

Probably the most troubling film I own is Clean, Shaven (I only pick it because I saw it last night...

Clean, Shaven

Good call on Dead Ringers. Beautifully made but ultimately quite depressing.

The "sloth" scene in Se7en freaked me out a bit in the theater and still does a little on DVD.

The Beyond has some truly whacked scenes. My advice is don't plan dinner around watching it.

The end of Hannibal.
I made the mistake of watching it for the first time on Thanksgiving, just after i finished eating
I was cool through the museum scene, had no problem with the pigs, but those last few scenes just turned my stomach.

Well in my collection it'd have to be:

*Cannibal Holocaust

*Cannibal Ferox

*Audition (Takashi Miike at his sick best. Though I hear Visitor Q and his newest take the cake)

*Red To Kill (HK CAT III. SICK.)

*Daughter of Darkness (HK CAT III. SICK.)

*The Untold Story (HK CAT III. SICK AND FUNNY)

*Ebola Syndrome (look above)

And the grandaddy of them all

*Exorcist (Watch the spider walk scene in the new version)

Blue Velvet. The whole character of Frank Booth is very disturbing- not to mention what he does. I also find Schindlers List to be very disturbing. I love both of these movies, two of my all time favorites, but they are both emotionally wrenching

Well...

The most controversial of them would be:

Braindead - (AKA Dead Alive) - Uncut, unlike the US versions.
The Exorcist -
House On The Edge Of The Park
The Last House On The Left
Natural Born Killers: Director's Cut

i own most of the films mentioned, however, for the most disturbing or troubling film i have is

I Spit On Your Grave

it turns me stomach just watching it, and i have only seen it once...which, honestly, is plenty.

Definitely Requiem For a Dream, though I do consider it to be one of the best films ever made.
