I'm not sure what's going on with this. I signed up and the email was sent immediately. Problem is the link in my "friend's" email is the exact same one Bandit already posted.

I sent four or five of these between my emails and my wife's emails and none of them have received a response. Hopefully we will get something or I feel like they could just be fishing for email addresses and we get nothing to show for it.

I don't remember, but I'm assuming I had to fill out the form in full. Maybe there was potential for abuse so they stopped letting people print out?

It must have been disabled because after I entered my email address then it basically went nowhere. It didn't give me the option to fill out a form. All that it allowed me to do was input my email adress and the email address of a friend.
