"Did XXXXX come with an insert?"
 

Any chance of stamping out these threads? Or at least consolidating them somehow? Perhaps someone many moons ago bought a [new] disc minus an insert, but 99% of the time the answer is "No."

I didn't get an insert in my copy of XXXXX.
