CSI Season 4 Rebate Question
 

Hi - since I saw a pre-existing question regarding the $15 rebate for CSI Season 4 I was hoping that someone out there may be able to help. I am in the process of finding my copy of the request as its been like 16 weeks and have not gotten any $$ yet. Has anyone who submitted the rebate gotten their check and how long did it take? Also, does anyone have who I can contact in the event I can't find my copy of the form to see if they even got my request?
