Anyone know what the participating theatres are that accept the ticket? I have a sealed one here, but if there are no theatres in my area that accept the ticket, I will just bring it back.

When my DVD from Amazon.com was delivered, unfortunately, I was 8000 miles away. I just got back early last week, and like many here, NO ticket.
Similar to fryinpan1, I contacted Amazon, telling them to either send me a ticket or refund me the cost of the ticket. Kudos to them, they did BOTH!
Actually, they sent me the following email:
Thank you for writing to us at Amazon.com.
I've confirmed that your order of "Resident Evil (Deluxe Edition)"
was supposed to contain a coupon within the DVD for $7.50 off a
ticket to "Resident Evil: Apocalypse." Unfortunately, we have been
informed by the manufacturer that some DVDs may be missing this
coupon. We regret any inconvenience this has created.
A toll-free consumer hotline has been established for "Resident Evil:
Apocalypse" Movie Money inquiries, and tickets will be mailed to
eligible callers. You will need to have the product in hand at the
time of the call and will be asked for information off the package,
along with the location of purchase (Amazon.com). The telephone
number will be in effect through Sept 24, 2004.
This number is:
866-607-9868
The day after I received this email, I received another email stating that I would receive a partial refund on the cost of the DVD. Not bad.
I received the Movie Money today, it is the exact one that SHOULD have been in the DVD. Unfortunately, it is still only good until October 1st. Lucky I got it when I did, otherwise it would have been a waste.

Mine had a ticket, and I am going to see it tonight.
It is pretty pointless to have a ticket sent out now (although it looks like the phone number is not in effect anymore).
