Man Who Fell to Earth Criterion blu Insert Needed

Can somebody out there offer up a scan of the booklet pages that came with the Criterion edition of this blu ray? Not sure how many pages it has, but the photo below seems to show it as fairly thin. I recently got a used copy without the booklet.

Yes, it's not there. Also put out a post over there as a request but no bites.

SOMEBODY must own this set ~ just need a volunteer with a scanner..
