Needed: Super Bowl XLI Cover Insert

I recently purchased a few DVDs which came without inserts/cases (previous owner thought they were a waste of space.) The only cover I am having issues finding is the one for the Colts Super Bowl XLI DVD. Anyone out there able to scan there's or point me in the right direction?
