LotR Case

I've had my Extended Editions of LotR for a while now and was going to go ahead and send in the form for the case that hold all 3. Well I lost the little form thing. Does anyone know if there is any other way to get it?

Well no, but the offer also ended on 3/31/2005 (or while supplies last). That was almost a year ago. I don't think they'd still have any left.

Or you could buy it Overseas. At DVDHeaven for US$3.05 for example.
