Wanted Metallica St. Anger dvd cover with insert
Use the same cover design as the album cover just bigger and clearly state its the dvd on the side and keep the PA sticker on it.

Here is my version of the cover. I had to come up with something a little different then originally planned. Didn't want to look like I was copying anyone. 
I'm going to work on an insert next. This should be uploaded to dvdcoverart by the end of the weekend.

Muya, if you could add the Parental Advisory and make the DD symbol a little smaller than I think you've got it nailed. Either way, it's awesome.
