$5 off "Standing in the Shadows of Motown" and "The Temptations"

I want to get the $5 rebate for buying "Standing in the Shadows of Motown" and "The Temptations" but I can't find the proof-of-purchase for "The Temptations." It's not on the front or back covers and it's not on the insert. And the rebate form isn't clear about whether you need a proof-of-purchase from both DVDs. And I can't find Artisan Home Entertainment's e-mail address anywhere.

Does anyone else have this problem?

I don't see a proof-of-purchase on The Temptations either. I bought mine at Best Buy, so they didn't charge anything for it.

I checked Artisan's web site (artisanent.com) and found a snail mail address and phone number. Perhaps that will help:

Same problem here, too. I just gave up and figured $5 for the Temptations DVD isn't too bad.

So, Best Buy is giving out INSTANT rebates for "The Temptations?"
Too bad there's no Best Buy in Hawaii.

And, yes, $5 is still a very good price for a DVD.

...or (if you don't want The Temptations) you can just order Standing in the Shadows from DDD for $12.22...

The sale they offered this week was to buy Standing in the Shadows for $15.99 and receive The Temptations for free.

I'm having the same problem. I photocopied the back cover and pointed it out to them, and figure I got everything so cheap, if it doesn't work, I'll live.
