The form that printed out at the register says that the "original UPC product code from the package" must be submitted. Do they want us to mutilate the DVD snapcase itself and cut the UPC out...or do they just want the FYE UPC sticker with the price on it that is on the outside of the plastic wrap?

I asked the guy at my store a while ago - he said to send in the original UPC from the box, but I'd seen someone on here had said that sending in the UPC from the price tag worked fine, so I tried it, and it worked fine - don't mutilate your DVD! I'd use the rebateshq.com tracking though - and keep an eye on it.
