Okay, book fans...I need your help (or, what book is this?)

A while back, I remember reading through a thread that I think was about someone's father passing away...I could be wrong. In the thread, it was mentioned that someone had bought their father a book that he could write in...a book that prompted the person to tell stories about things or times in their lives, so their memories would live on. Someone else had chimed in saying they had gotten their dad the same thing as a gift.

Maybe I'm wrong...it was a long time ago and I'd have a hell of a time finding the thread.

Is there a book out there like this? I'm thinking about getting it for someone as a gift, but when I went into Borders, I just had no idea where to look.
