Mine is Brink. It sounded like such an awesome game and had a kickass trailer. I bought it for $10 used a few months after it's terrible reviews and dumped it after playing for about an hour. Awful game.
Dead Island was disappointing as well...after getting really good reviews, it was just really boring.

Battlefield 3. Bad Company 2 is the all-time best online game ever, and BF3 makes zero improvements and hundreds of problems. This could actually be my biggest disappointment of all-time.

Rage for me. I usually keep games forever until they're worth nothing but this game I returned as soon as I beat the campaign. Graphics were awesome and the game play wasn't bad but the story sucked and the ending was the worst I've seen in a long time.
Homefront would be another I'd add to the list.

my biggest disappointment is spiderman: edge of time. was nothing like shattered dimensions.

Dragon Age II. It was clearly rushed out the door and as a result it ended up being not only a half of a game, but a repetitive and dull on with that, that also featured an inexcusable amount of bugs and glitches, including a few game breakers.

Arkham City - not that it was bad, it was actually really good, but I was just expecting it to be way better than Asylum and not only was it not better, it wasn't even as good. It's all about expectations.

BF3 has been growing on me despite my early distaste. LA Noire was a good game but grew tiresome. Arkham City on the other hand feels like Grand Theft Gotham and that has been a big turnoff. I played 30 mins or so and I just haven't gone back yet. Arkham Asylum is a favorite game of this gen. There's nothing terrible about it... It's just not what I wanted.

Every game that isn't Skyrim.
Also, I haven't played the new Spider-Man, but I really liked the first, so the terrible reviews of the new one are pretty disappointing.

Batman: Arkham City for me as well. It wasn't a bad game by any means, and it did correct some problems from the first game -- but overall the experience wasn't nearly as amazing.
