your favorite "DEATH" on Six Feet Under (show opener)
I guess my favorite would have to be Bruno Baskerville Walsh who, in 1972, jumped off his 12-story dorm roof while under the influence of LSD. The girl he's with (Sandy/Cindy) is starting to get scared and suggest they go back down to the party, but Bruno says, "can't, gotta go," and he runs and leaps off the roof, to land on a car.

The escaped love dolls floating in the sky and how that Christian lady thought she was seeing "The Rapture" and got out of her car to go run into traffic, only to be run over.

My favorite was the woman celebrating (her birthday?) hanging out of the sunroof in the limo, and doesn't see the low ceiling coming. Some great makeup work on that one!

For some reason the one that sticks out most in my mind was the old man who got up, dressed in his best suit, drove to the funeral home, left an envelope of instructions for his funeral on the dashboard and died.

I really like the one with the guy in his SUV that opens his door to get the newspaper, falls out of the car and runs himself over. Also another head death - a recently divorced woman that Titanics out of the sunroof of a limo and connects with a cherry picker.
I will say that the office massacre is the worst in terms of 'realism'.

Probably when the Mountain Lion mauls the guy running in the hills. That one was simply fantastic.
Another memorable opening was when the boss of the conglomerate funeral home group (the one that continually attempts to purchase Fisher and Sons) is playing golf. As she pontificates about something or other, she abruptly and violently grabs her chest. The viewer is led to believe she is going to have a heart attack and die. She quickly recovers, then shanks a tee-shot far right off of the fairway and into an unsuspecting homeowner's temple.

maybe it was a different one because i could've sworn there was one where someone (lady?) got her head ripped off by an overhung light on street. could be mistaken though. need to see the series again soon.

I have two: The one where the guy was cleaning the giant blender that was accidently turned on as well as where the woman is making her husband breakfast and he keeps talking on and on until she finishes making the food, hits her husband over the head w/ her frying pan and then sits down to eat the breakfast herself.

I don't know if it's a favorite, but the one I remember most often is when the husband and wife were arguing, and she fell on the poker for the fireplace. I think they were having some meaningless argument, and then that happened. I can't remember the context in the episode, but I do remember the scene itself.

I think the mountain lion jumping into screen from seemingly nowhere has my vote. Since no one's mentioned it, how about the guy that got sliced mid-waist by the elevator? Huh? Huh? Am I right?
