DVD Talk review of 'Lost: The Complete Fourth Season' (Blu-ray)
I read Jeffrey Kauffman's DVD review of Lost: The Complete Fourth Season at http://www.dvdtalk.com/reviews/read.php?ID=35542 and...it mentioned an insert, but my copy didn't come with one.

Copies sent out for review apparently had an insert tucked inside -- writers for other sites have reported the same -- but it was left out of the retail version. Buena Vista is reportedly mailing inserts to people who request one, though.
