Landstalker and Beyond Oasis are two big action adventure Genny games.
Landstalker-Kinda like the Alundra games on PSone
Beyond Oasis-Kinda like Zelda with an Arabian theme
I own both but haven't really played them yet. I got them in a bundle a few weeks ago. Here's some reccomendations though:
Streets of Rage 1/2- Possibly the best Beat'emup ever. Only Turtles in time comes close.
Sonic 1-3 and Knuckles- No Genesis collection is complete without these. 2 is my personal favorite and possibly the hardest. 3 has a save feature which is a first for the series and genny platformers in general (because the game is so damn big)
Shinobi series- Part 3 is my fave only because they refined the platforming gameplay in "Revenge". These games are tough but thuroughly redeeming.
Those 3 are going to be your main reasons for owning a Genesis. There's something for everyone though as seen here:
Action: Gunstar Heroes, Contra Harcorps
Puzzle: Robotnik's mean bean machine, Klax*
RPG: Phantasy Star series, Shining Force series
Shooter: Lightning Force
If you ever pick up a SegaCD, I've got plenty of suggestions for that too.

Oh yeah, Flashback was good (for the PC, anyway.) If you like action puzzlers - games like Abe's Oddworld - then this is for you.
Flashback was created by the same company who did an older game called Out Of This World that was an awesome action puzzler. They made a sequel for that game exclusively for the Sega Genesis called Heart of the Alien, but I don't know how that is. Should be fun.
I've heard a lot of great things about some game called something like Night Into Dreams, but I know nothing about it. Anyone have info?

Another vote for Toe Jam and Earl. Team USA Basketball was good too. The teams from the other countries actually played as well as the USA team in the game.

One of those ultimate cult games. I've only played it a bit years ago, but I liked it.

Please note that Nights...into Dreams is for the Saturn only. A completely different system.
As for the Genesis, did anybody else like Vectorman?

Still more votes for
Toejam and Earl
Landstalker (like Alundra but better)
Shining Force (any you can find)
Streets of Rage
Gunstar Heroes
Another game I loved was Road Rash 2, it was one of the first driving games I played where going the wrong way could give you better jumps than going the right way (you died after, but massive jumps)
