The Japanese DVD has a commentary track that includes both the director and the producer, so it's a completely different track than what's on the US DVD.
The Japanese DVD also has quite a few extras, and an anamorphic transfer of the uncut version of the film.

The only downside is that the audio is only DD 2.0 surround, so the German version wins there.

I haven't heard of any new SE. The Fox R1 out now is pretty decent, excepting for the non-anamorphic image. It contains DD5.1 sound, director's commentary, and deleted scenes, all for under $10.
Sure, it's the R-rated version of the film, but it doesn't bother me too much that 11 secs were cut from the film. If you check out the deleted scene 'Funny, Funny' on the R1, you can watch it in it's original uncut version and compare it with what's in the film. I didn't even notice it was a different cut until I closely compared them.

The Scandinavian version is not anamorphic and the sound is only in 2.0. The company that originally released it (Scanbox) is currently working on a newer version that will be released later this year. Hopefully it will have DTS as well, as Scanbox has started using that on most of their recent releases.

It's pretty hard to get this movie from germany, since it's not allowed here to sell "FSK18" movies on the internet (due to JuSchG -> Laws for the proctection of juveniles), that is mainly pornography and violent movies. Usually you have to go into a store where you can show your ID and confirm your age before you can buy this kind of movies. It's a different story in Switzerland, there you can buy FSK18 movies without any Problems on the net, but they charge really high prices (for the DVD AND for the Shipping - even to germany). So I guess the safest bet is Japan or Switzerland if you want to have the DVD.
Oh, by the way, the disc doens't have any english subs (for the ones who might need 'em).

According to dvdcompare, the German and Japanese commentaries are different -- the German is the same as the US one? I guess as a completist then, the best option (hardly the cheapest) would be to go for the German version and the Japanese version.
Not that it'll be the first time. I have the R2 Japanese Mad Max box, + the MGM R1 SE. Some movies are just worth it ;p

You can't buy it from Germany, since it's illegal for German stores to sell movies rated 18 online. That's why you gotta buy it from Swizerland, for example.
If you can wait two months, a Scandinavian special edition release will be out. Probably with DTS.

The R2J commentary on Naked Lunch is different than the R1 version too.
