Adaptation and 25th Hour have no insert?
Got "Adaptation" (in a cheap looking semi-transparent case) and "25th Hour" from Columbiahouse and both have no insert. Is it usual?! At least I thought Columbia Tri-star's release should all have insert...

I didn't think Adaptation was "cheap looking" at all. I kind of like this style - it reminds me a bit of the 3-disc Canadian Pianist that came out last week. Although the transfer isn't so good, the packaging is awesome, and the extras disc and soundtrack make the set worth having.
Back to Adaptation though, I wish more studios would make packaging such as this...

My Adaptation had an insert. It was a memo to someone telling them to find some cases without the clips for an insert because they didn't have anything to clip there.
I liked how it was just wide enough to cover the chapter listing. That way I didn't have to worry about seeing spoilers.

The thread on inserts was already mentioned above and we also have the Adaptation case discussion here, which well covers the insert.
