The most intense start to a metal album.
I never ventured too far out of 70's & 80's Heavy Metal and the 80's thrash big four until recently when I picked up Vile by Cannibal Corpse. The opening track "Devoured by Vermin" is the most intense thing I've ever heard. It blew my mind.

Can anyone match or surpass that? That shit floored me. Feel free to post any "heaviest" songs.

"Battery" from Metallica's Master of Puppets. Starts out acoustic, but the first time I laid the needle down and started spinning it, I know it was eventually going clamp my balls like a bear trap.

The Hellion/Electric Eye would be my first answer as well. We Rock, Over the Mountain, and Aces High also kick some serious ass right off the bat.

If you want to step outside the metal realm and into punk, this scared the crap out of me when I was 13. Try listening to it with the lights off ....
