Anyone have Adobe Acrobat (full version)?

If so, can you please download this form from the Missouri Dept. of Health web site and tell me if the fields are editable. If possible, I'd like to be able to complete the form on the computer and then print it out. You'll have to excuse my ignorance if the "editability" of the form would be perceptible in Acrobat Reader as well. Thanks.

There aren't any defined fields, even in the full version. For future reference, Adobe Reader will allow you to fill in fields in a PDF; you just can't save the document.

Crap, I think I knew that. Thanks. By the way, does Acrobat have a quick and easy conversion feature for a form like this, or would it be a lengthy process converting the fields to defined?

It would be a pretty time-consuming process as you'd essentially have to build it into form format. Unless you need to fill it out a bunch of times you'll find it much quicker to just print it out and hand-write it.

You may have some luck w/ a scanner and OCR (optical character recognition) which would allow you to type in it (either via Word or another compatible program). Again, that's if you have a scanner & OCR software.

Send me an e-mail message (Missouri pdf in the subject line) and I'll e-mail you the form in a fillable format. I downloaded it and converted it to a fillable pdf. It only takes me about five minutes to convert a form to fillable.
