Afterlife (Dead Like Me/Pushing Daisies/Wonderfalls crossover spin-off) - Combines reapers, the Pie-maker and the girl that can talk to inanimate animals.
Barney Stinson: Hard Time (How I Met Your Mother spin-off) - Takes place after Ted tells the kids how he met their mother. Barney has undergone a deaging process and has decided to be the first to travel through time. Of course it doesn't go as planned and we follow him as he shags his way through time, trying to get back to the future.
Dutch (The Shield spin-off) - Holland "Dutch" Wagenbach is a detective by day and a blossoming serial killer by night.
Faith The Vampire Slayer (Buffy The Vampire Slayer spin-off) - Faith Lehane on a motorcycle getting into adventures.
Fray (Buffy The Vampire Slayer spin-off) - Melaka Fray must slay lurks (a.k.a. vampires) and other nasty things in a futuristic New York City.
Hurley's Island (Lost spin-off) - With the Others wiped out and the island's mystical energy rendered inert, Hugo "Hurley" Reyes purchases the island in order to open a resort and hilarity ensues.
Mirror Image (Twin Peaks spin-off) - Special Agent Dale Cooper, on the road, hot on the trail of a serial killer, himself.
Ripper (Buffy The Vampire Slayer spin-off) - Rupert "Ripper" Giles in the UK solving supernatural cases.
Veronica Mars, F.B.I. (Veronica Mars spin-off) - The adventures of a spunky F.B.I. agent who doesn't go by the book.

I also liked the notion of The McPoyles, that's the only show I could see even coming close to par with It's Always Sunny.

I would have liked to have seen "Crusade" (the Babyoon 5 spin-off) given a shot without TNT doing everything they could to kill it.
And "Faith" and "Buffy the Vampire Slayer Animated." I would have loved to have seen "Faith" be an FX series with a harder and darker edge than either "Buffy" or "Angel." I can't believe that nobody wanted the animated Buffy series, considering all of the animated crap that does get made.

Lupo & Bernard: Just like this season of Law & Order, but without all that courtroom shit.
Incarcerated Development: George (and Oscar) Bluth's adventures in the California penal system.
Two Men: Russell & Jeff from Rules of Engagement, free to be funny without the we-see-that-coming-a-mile-away jokes from the young couple. Warburton & Spade already carry the show, just make it official.
It'd be cool to give someone from Laguna Beach a spin-off. And someone from that spin-off a spin-off. Then spin-off a spin-off from the spin-off's spin-off. Then have someone from Laguna Beach come back and just try to connect the dots.

Hurley and Sawyer circa Season 4 dharmaville, sitcom but it's just their characters from that period of the show but nothing to do with the Lost mythology (i.e. they live in the states and share a house.)

I'd love to see a spin-off that shows what hppened to the characters from the 4077 after M*A*S*H ended. Maybe focus on the secondary characters to give the show a different focus.
I think Matt LeBlanc's character from Friends could carry his own series. I'd love to see them spin that off.
George Lucas did a TV series about a boy growing up around the turn of the century and interacting with all sorts of historical people and events. During one episode, they flashed forward to the adult version of the character, and I'd love to see more stories about that adult version. They might work better as a series of movies instead of a TV show.

Chekov: A spin-off of the new Star Trek movie/Original series. After a disaster that flings the Enterprise into a new alternate universe (this one inhabited by the characters of the first three Star Wars movies). All of the cammand crew are dead except Chekov who takes command of the Enterprise. He wants to help the rebel alliance and they would like his help because of the ship he commands but because of his horrible accent they have a difficult time communicating with him and in the end just cant take him seriously. He tries to take on the Death Star alone and ends up getting the Enterprise destroyed.

In the DS9 Tribble episode, Sisko was visited by "temporal police" from the 29th century who were supposed to ensure no changes to the timeline. I always thought that would have been a much better idea for the next Star Trek series rather than Enterprise. That way you could have "future" and "past episodes".

How about a spin-off mash-up involving the romance between Lorelai Gilmore (of Gilmore Girls) and Casey McCall (of Sports Night), co-written by Amy Sherman-Palladino and Aaron Sorkin?

Star Trek:SFCIS. Just like NCIS, but set in THE FUTURE!
Or pretty much any show set in the Firefly verse. Either another ship and crew, or stories told from the governments pov.
