Going away from horror, I would make a vote for the ending of the Warren Beatty "Heaven Can Wait". Profoundly bothers me every time I think about it.
In the other direction, "Field of Dreams" especially a first time viewing can just be brutal and linger for far too long.

Crumb and American Movie.
Crumb because R. Crumb's family is so fucked up. His kids seem well-adjusted, but his brothers seem really pathetic. (I think his brother Charles committed suicide a while back).
American Movie because the two guys trying to make the movie remind me of the weirdos, burnouts, and losers I went to high school with. Not too bright, lots of ambition, and zero ability to make their dreams reality.

A movie has never haunted me. But I know several people who have wished they hadn't seen certain horror films. Years ago, a few of my relatives on my dad's side of the family said that they wished they never saw The Exorcist. Last year, I was visiting with a maternal cousin and her husband. Her husband was telling me about how he lived down the street from a theater as a kid with his family. He was 13 when The Exorcist was in theaters. He and his brother went to see at night. When they got out of the theaters, they ran home. He hasn't seen that movie ever again.

I enjoyed that movie and that scene was one of the best "reaction" moments I've ever seen in a theater, laughed til it hurt (turns out that was more or less the reaction they were going for too, so, woo.)

Ok for me, though not sure if I regret any of them but they still haunted me, big time.
- Miracle Mile: The ending
- Jaws: Duh
- Saving Private Ryan: Knife scene and first 15 mins
- Mulholand Drive: Many scenes though the "restaurant scene" steals the cake
- Twin Peaks The Series: Bob and the couches. You who have seen it know what I mean. Plus countless other scenes such as the last 20 mins of the finale.
- Creepshow: The story with the swimmers stuck on a raft in the lake. Pretty much that whole story as growing up, we would spend every weekend at my family's cottage which is on a lake. Fuck!
- Funny Games: I never saw the original but the end scene with Naomi Watts, ugh. Total helplessness.
- Cache: Actually a great film but the whole movie was just totally creepy and has made me feel someone could be watching your every move and that person may be from your past and while you have forgot about them, they have not forgotten about you.
Those are just off the top of my head, but I cosign many already noted here for sure.

As a kid no movie freaked me out more than Gremlins. I was certain those fuckers lived under my bed and were going to fuck my shit up after midnight.
How is that supposed to be played for a laugh? Serving one his own brain and slowly losing coherence because of missing brain is just demented. It also has enough of a root in reality to make it that much worse.

Not so much as haunting, but the ending had me grossed out long after I had finished watching it.
I walked out of Evil Dead (2013) after seeing the dead animals. It bothered me.

I can't explain how strange I felt after watching Brazil when I was 15-or-so. So many new feelings.
I'm glad I watch it and have watched it many times since.
The first time I watched it, it was on Sci-Fi channel, and it was the director's cut.

Hellraiser and Hellbound. Watched these as a kid, and they seriously freaked me out. Anatomical drawings always gave me the willies, and these movies both featured skinless people running around killing people. I still sort of get freaked out thinking about Uncle Frank hiding in my closet.

The stuff that bothered me seems mild compared to the descriptions here, but my excuse is: I saw them when I was young.
Wait Until Dark (1967)—Found it very unnerving how Audrey Hepburn was so helpless.
The Legend of Lizzie Borden—a 1975 TV movie with Elizabeth Montgomery. Very graphic at the time, considering it was broadcast television (ABC). "Are you a good witch or a bad witch?"
