The first R for me was when my mom took me to see The Punisher. There were many other R-rated films my parents took me to see cos they felt I could handle it (though my mom was more conservative - "close your eyes" - than my dad a few times). The first R I remember seeing alone was Army Of Darkness but I'm sure there were others because I remember seeing Evil Dead 2 and A Nightmare On Elm Street andmy parents reallly didn't like me seeing those type of films (though the Nightmare films seem to hold a special place all their own in my mothers heart).
I don't remember my first R video cos I think I saw them all when I was growing up.

I think the first one that I saw in the theater was Blue Thunder. I went with my dad's friend and his son (who was quite a bit older than me). I remember watching the scene with the nude girl doing stretching exercises and liking it quite well.
I think the first R rated movie I saw ever, though, was H.O.T.S.. My friend's dad had it on tape so he (my friend, not the dad) showed it to me and I was like, WOW.

I got into slasher movies a couple years later but didn't see many other R's until I actually was 17.
Not unless you count watching 'ON' TV (early pay TV service) between the scrambler!

It's an unscientific sampling, but the two most popular first R-rated movies seem to be Robocop which was listed by 4 people, and Alien which was seen by a whooping 6 people.
Also, I envy anyone who got to see Psycho back when it came out.

The first I can recall in the theater was Tarzan, the Ape Man when I was 8. My two older cousins (2 and 4 years older) and I got our grandmoter to take us, and she was not as happy as we were with the nudity.

National Lampoon's Animal House.
For some reason, my dad felt it necessary to take me and my brothers to see it when it played at a local theatre. Funny thing is that he laughed loudly through the whole damn thing, and then while eating afterwards, kept shooting it down for being "just okay."
On an slightly interesting note - yesterday, I ran into a local movie listing from the time while researching a project at work. It listed the movie playing at the theatre I saw it at, so I can even pinpoint my first R-rated flick as being in August of 1978. Also playing at the time: Foul Play, Heaven Can Wait, Star Wars, Grease, Hooper, The Norseman (with Lee Majors), International Velvet, Hot Lead and Cold Feet, and others. Strange seeing a movie listing page with no multiplexes and a ton of drive-ins (only two remain in MD).

My dad let me watch "The Blues Brothers" on cable. Yeah, a pretty tame "R", but my mom was mad nonetheless.
Also, several years later, he got in trouble when he let me rent "Robocop".

All that Jazz is the one I remember seeing when I was 6. I remember all the nudity, the tent I pitched, and wondering why the guy "wet" his pants after licking the women's boobs. My parents used to take me the R movies all the time. Guess it was cheaper than a babysitter.

I think it was T2 when I got the video as an Xmas present. Loved the movie ever since, even though now I think the movie isn't a very hard R; it's actually pretty mild to me.
In theater:
I'm not really sure, but if I had to guess, it would be Starship Troopers. I would've been 11 at the time. Being a Paul Verhoven movie, that was probably the wrong one to see, from a parenting standpoint.

Aliens was the first R movie I saw. It literally scared the crap out of me so bad, I had nightmares for about a year afterwords. I clearly remember the night I got home from the movie, I kept on having this re-occuring dream where I was walking in this hazy maze and there were aliens coming for me. I was really really scared.
