...you say it's been released in Japan - I couldn't find it on the (only) Japanese site that I've ever used - maybe you'll have more luck!...

There are a bunch of DVD box sets available.
ER first, second, third, fourth and fifth seasons are each available in box sets at around $150 per set. I think each box contains around seven discs - all region 2, NTSC, bilingual.
The ER East/West Coast live show is also available on single DVD.
Not sure about online but all the stores here have them!

Yup, you're right! I forgot Amazon Japan just launched DVD sales. I just checked the page and they offer international shipping too, 600 yen for the first DVD and then 300 yen for additional discs.
