Correct me if I'm wrong, but doesn't this not really happen anymore due to monitors? In behind-the-scenes footage, I often see people looking at playback monitors with both a square and a rectangle on the screen; unless I'm mistaken, I thought that was so the picture could be properly framed for both widescreen and fullscreen, and avoid showing boom mics, lighting and accidental male/female nudity.

People often erroneously use Terminator 3 as an example of a recent movie with extra boobage in the open-matte transfer, but in fact there is no true nudity in any version of the movie. The actress's breasts are covered by hair. All the full-screen disc offers is some extra cleavage.

"Who's Harry Crumb?" -- during the opening credits when the woman is getting ready for the spa, in the FS you can see her nipples in the mirror (at least in the foreign version, see link). Noteworthy because this was otherwise a tame PG-13 movie.
