Did people that bought the Ben Stiller Starsky and Hutch DVD receive a chapter listing inside the DVD?

Nope, no insert. I was a bit confused by your thread title, a slip case/cover is like a cover that goes over the outside of a case, like Once Upon A Time in Mexico. There's also a subforum all about inserts linked at the top of the forum.
