I count all of the HBO's and other's with multiple channels as one. On-demand counts as separate from original channel.

All the premiums I have from DIRECTV include their respective streaming services.
The HBOs, Cinemax's, Starz and Showtimes all have multiple channels on DIRECTV. So I don't feel the need to list all of them.
I know there's more out there, but this is all I have time for and can afford.

I have access to Showtime, Cinemax, Starz, etc. the premium channels in Comcast.

Netflix and Amazon. If you're counting WWE, I have that for my son.
No premium movie/pay channels on cable beyond our HD tier which is still a bunch of duplicates and chock full of shit channels we never watch.
