Do you use a geeky video game ringtone or notification sound on your phone?
I am currently using the World of Warcraft sound effect/music when you turn a quest in with my text messages. I like it.

Nope, I actually use the default Google notifications right now. I've used random ones in the past though, like the FF7 fanfare.

For a brief time, and then I realized how annoying any sound emitting from a phone is annoying.

I used to have my laptop boot-up sound be Buster from Arrested Development saying, "HEY, Hermano!" and my laptop shut-down sound be GOB saying, "I've made a huge mistake." The second one was because I found that the laptop would be in the process of shutting down just as I was remembering something I should have done beforehand and then, too late. It's actually happened where I said the words right along with GOB for legitimate reasons.
I haven't used bootup / shut-down sounds in three years, though, because it can be distracting in class. Also, I have never, ever, ever, ever, ever used a ring tone on my phone. Always on vibrate.

I've always wanted to add special sounds to my phones over the years, but am much too lazy to actually do so.

Nope. Default ringtone here as well. Never bothered with trying to find somewhere to download others.

I'll join the club. I have the Zelda secret tone for text, and the Mario theme as my ringtone. But, I always mute it if I'm in a meeting.

No. Never. Mine always has been and will always be on vibrate. Ring tones are annoying - especially so in inappropriate locations (any place other than your own house).

90% of the time my phone is on vibrate, but when it's not, the MGS "Alert!" sound clip is for my text messages, and the "Codec" beep is for calls.

Not video game related. I have used the theme song from Sledge Hammer as my ringtone and Mr Burns saying "pish posh." Though, I too usually have my phone on vibrate.

I've used all kinds of sounds for my phone. Main one I like for Voxer notifications I use the MW2 power on sound - a chirp when the Infinity Ward Logo appears.
For text notifications I've also used Sonic Rings, Pac Man Start, PacMan dying and the Skyrim level up sound.
Not video game related but When my wife calls I use the Game of Thrones violin Theme

Right now I'm using the musicbox tune from The Conjuring as my ringtone. It was creepy back in October but not so much now..

The achievement unlocked sound is my text message notification. Haven't bothered to change anything else.

My text message is the Minion from Despicable Me saying "oh, hello."
My ringtones are all stock iPhone ringtones. I haven't had a custom ringtone in years. I hate those people that have songs for text messages. So every time they get a text, you hear an annoying 30 second song clip.

My Voicemail sound is "hailing frequencies" sound from Star Trek

I haven't set it up on my new phone yet, but my old text notification was the Sonic ring noise. That setup actually lead to confusion when I was in a Target and the display game on the PS3 or 360 was Sonic 4... thought I was a tad more popular than I actually was.
And while not strictly video game related, my ring tone was the AOL dial up noise and my voicemail message was George Takei from the Halloween episode of Community where he created a special voicemail message for anyone named Kevin (I.E., me)

Not currently, but in the past I've used either the codec from Metal Gear Solid, the Turret Serenade from Portal 2, and the death theme from Super Mario Bros. (for my old job, because whenever I got a call from there I felt like killing myself).
