She recorded it and likely leaked it herself (which she is denying), or she gave it to someone that she knew would leak it.
That's why she's so calm through most of it - IMO, she's using it to ruin Mel. She set him up and he delivered the insanity on cue.
I feel sorry for the guy. He's clearly very unhappy and doesn't seem to know how to deal with anything difficult. That doesn't excuse his behavior in the slightest, but his issues may go way back.
She wanted to get at him, and she got him. He, of course, stupidly took the bait and predictably let his anger take over. What a pair.
Mel just needs to take some time and get some help. His career is toast regardless, but he needs to get help otherwise he will not end up well.
I wonder how many, "Ghoul Pool," or, "Dead Pool," lists he'll end up on in January...
