Does anyone have the Coconuts Seinfeld Rebate form?

Coconuts took down the rebate form for the Seinfeld DVDs very quickly. I should have printed it out when I ordered the DVDs, but I didn't. I tried contacting customer support, but they are unresponsive. Did anyone here by chance save the form?

It will only frustrate you. I purchased 3 movies at the same time that were had a coconuts discount. I figured they were cheaper than they'd be at DDD during the 20% sale.

That as back at the begining of September. To date I've only gotten one of the three rebates. I won't be purchasing from Coconuts again. It doesn't matter who is in charge of the rebates - in the end I purchased them from Coconuts for the rebate and I didn't get the money back so it is Coconuts who is to blame if the outsource to an unreliable rebate center.
