Standard Jedi Training

I have a few questions for someone who is into the EU.

How are the children found that are brought in to train with the Jedi?

Do their parents notice something in them and bring them to the training or are they discovered through the force by the Jedi?

Lastly, do they lose all connection with their families once they are accepted by the council?

Just going from what was in Knights of the Old Republic (an RPG game) once they begin Jedi Training they are not encouraged to associate with family again as that provides a distraction in their training, a distraction which could allow dark feelings and the dark side to creep into their lives.
