New, 30-40 bucks.
Used, 15-25 bucks.
No monthly fee for access, except for MMORPGS [Everquest and FFXI].
I don't play online often, but last time I did [Ratchet and Clank] I had no problem finding people to play with. It'll depend of course on what game you're playing and what time of day.
Never had any technical difficulties, setup was a breeze.

Why don't you make a few more threads about this?
The adapter costs about $40 new, I got one for $20 used.
Most games don't have any recurring monthly fee. A lot of games don't have very many people playing, sometimes it's in the single digits. Most of the newer more popular games will have enough online for you to find a game. It's better if you have a friend or friends to play online with.
No technical problems aside from a few random disconnects here and there.

I got mine free from Sony for Beta testing them back when they first came out,but i have never actually used it since i switched to DSL right after the Beta period and never could figure out how to get the settings right to make it work with DSL (probably need a router?).I am not the most tech-savvy person when it comes to that part of it.
