Most make fun of me, but b/c I was lost, in the NJ Pine Barrens I can totally relate.
By far it's The Blair Witch Project
Ju-On might have the scariest images.

Second for What Lies Beneath, that one creeped me out for a couple of days (except the ending was kind of cheesy IMHO).
That's the only one I can think of off hand that I really got a lasting reaction out of. Lots of movies w/ "jump" scenes but nothing w/ that lasting effect. Although Cabin Fever came close, atleast it kept me thinking for awhile, more in the sense of thinking about how it could actually happen/how people would react in a similar situation, but I wouldn't really call it "scary".

Favorite: Audition
Scariest: A Tale of Two Sisters (not scary per-se, but it made me situp and take note)
Most disappointing: Ju-on: The Grudge

I guess it is really interesting seeing what people find to be scary . . . for some people it is jump and and say boo style suspense, for others it is grind house gore. Personally, I think that it is a clever story put together with creepy visuals and the right atmopshere. That said, there hasn't been much in the last 20-25 years!
I'd actually have to somewhat agree with Cabin Fever. But not for the splatter fest that it is. While I wouldn't call it terrifying, I would say that it really gets in your face and confronts you with the atrocities that people are capable of when the chips are down.
Doesn't somebody need to say "Showgirls"?
28 Days Later? Did ANYTHING happen in that movie?
I found Session 9 to be very atmospheric and creepy . . .
I hate to say Dawn of the Dead (2004), but I would nominate it.

SCREAM fucked the whole genre up.
Scream did not fuck the genre up. It fucked up the slasher genre up, but not the horror genre itself. Why I love Scream and Scream 2, every slasher film that came out after it tried to become "hip." I Know What You Did Last Summer was okay, but the sequel was a piece of shit. Urban Legend was nothing special, but the sequel was fucking terrible. And speaking of the Urban Legend sequel, who the fuck was the genius who casted Joey "Whoa" Lawerence in it? Christ oh fucking mighty. And I'm sure there are a slew of direct-to-video films that tried to take the Scream feel, but they all probably sucked. Mostly for the fact they went direct-to-video.
Then we had Warner Brothers and Dark Castle's fest of shit. House on Haunted Hill? Thirteen Ghosts? Ghost Ship? Gothika? Someone please kill me.
Now not only did Scream in essence fucked up the films trying to imitate it, it also fucked up quite a few sequels to some classic slasher films. Jason X is a schlocky piece of cheese that I happened to enjoy, but it didn't feel like a Friday the 13th film at all. Halloween: H20 was down right fucking terrible, mostly for the fact it felt like it was pointless and did simply not to be. And Bride of Chucky? A complete disgrace to Child's Play 3, which in essence, was a piece of shit itself. Freddy vs. Jason? Terrible film, but it least it had some titties on display. What can I say, I'm a simple man with simple needs. And then we had Scream 3. A film which became everything that Scream and Scream 2 made fun of.
Now we're at the point that Hollywood needs to remake classic horror films. While The Texas Chainsaw Massacre remake was terrible, I did love the Dawn of the Dead remake (which felt more like an addition to the original film, and not a blatent remake).
Now the horror genre itself? The horror genre has always been hit-and-miss. You have good horror films, and you have shitty horror films. There is never an in-between.
Asia (specifically China, Korea, and Japan) has been pumping out some great horror films lately. The Eye, 2LDK, Suicide Club, the Ring trilogy, the Ju-on series, Versus, Wild Zero, Junk, any film by Mr. Miike, and so on and so on. Right now, the Asians are the only ones coming out with honestly good horror films.
We also have the British coming out with some solid flicks. Dog Soldiers, 28 Days Later, and Shaun of the Dead all come to mind. Hell, may I even suggest the French with Brotherhood of the Wolf and Haute Tension?
But back to American horror, yeah, it kinda sucks. As I mentioned earlier, the remake to Dawn of the Dead was great... but that's been about it. I enjoyed May to some extent. I liked Ginger Snaps. But yeah, I can't think of anything else. Open Water sucked. Session 9 was terrible. American Psycho took a shit on the magnificent novel by Bret Easton Ellis. And anything M. Night does is fucking awful (except Unbreakable, but it's not a horror film).
So the whole point to this post was that while Scream fucked up the slasher genre, the horror genre itself is still going strong... but not in America... open your eyes to foreign films, and you'll be surprised.

The Ring is one of the only movies I was really scared of by the time the movie ended. I watched it with two of my friends and they were visibly shaken, so naturally I grabbed my cell phone and dialed their number promptly after the credits started rolling.
Another good one is the Blair Witch Project, it is getting that time of year when I turn off all the lights open the windows to set the mood for my yearly viewing of Blair Witch around Halloween.
