If you are talking about the 2 disc Central Park Media edition,
it does NOT come with an insert

The piece of paper or a booklet inserted into the tabs on the left side of an opened DVD keepcase.

It doesn't come with an insert, but all the information (chapter selections and what not) are on the back of the cover as the cover is see-thru. ;)

Did CPM releases a 1-disc version with a different cover? I saw it at Target.
