DVDs that look like props from the movie.
i.e. the packaging, aside from having the name of the movie/show, incorporates the rest of the box in the world of the movie.
I know there's a lot, but the main I can think of now is Memento.

Evil Dead 1 and 2: Book of the Deads
Reservoir Dogs: Tin Can (new release)
Total Recal: Mars Tin
Planet of the Apes: Ape Head Bust
That's all I can think of for region 1, region 2 has a lot, lot more to offer...

I guess the "Cylon head" packaging for Battlestar Galactica would qualify, wouldn't it? Of course that's not a movie, but the pilot started out that way.
As a side note, I'd imagine a "Cylon head" package for the new series wouldn't be nearly as interesting. Just one of those old "do Barbie's hair" heads, I guess.

There are a TON of customs out there with this theme. Some are great, some not so much, but man they look good in the collection.
A few good ones off the top of my head:
The Ring: cover designed to look like a VHS tape with only "don't watch" on the spine
Cast Away: designed like the FedEx box in the movie
Goonies: replica of the treasure map
One Hour Photo: designed to look like the SavMart photo envelope
People Vs. Larry Flynt: looks like a Hustler magazine
they can be found at the usual places.

Just saw a King Kong (2006) set that looks like a film case. Pretty sweet...

The original LE packaging of Basic Instinct came in that clear case with the ice pick. Also, there's the Japanese Alien Quadrilogy set that came in that giant alien bust.

Pearl Harbor: Director's Cut resembles a WWII era manifest book.
