Got the idea from a recent Entertainment Weekly.
I think the cliched answer would be "The Sixth Sense", but I don't care that is mine because it truly blew me away. But a very close second would be "The Usual Suspects"

This not #1 twisted ending, but some of my other movies have already been listed:
Presumed Innocent (1990) Harrison Ford - get build up to a gentle ending
No way out - Kevin Costner

I'll try to add a few that haven't been mentioned yet. All of these have very cool twists, both obvious in your face and some that are more subtle.
Adaptation
Eternal Sunshine of the Spotless Mind
Fight Club
Memento
Oldboy
The Others
Se7en
Sleepaway Camp

-usual suspects, lol yea!
-i guess the ending to NARC is a twist, I liked it alot anyways.
-I've watched empire so much that I forget that the twist is even a twist at all. But a good one I guess.

Playing with people's knowledge of Psycho, I thought it was really surprising in Psycho II
I'm a little confused, what would you consider the 'twist ending' of American Psycho?

Basic Instinct-maybe a little predictable
Jagged Edge
I think it was Robert McKee (the screenwriting expert and man played by Brian Cox in Adaptation) who said you never have a thriller unless you have a false ending.
